NOTICE OF HEARING ON DEATH BENEFITS
NOTICE is hereby given to any and all parties who claim any interest in or entitlement to benefits under the Workers' Compensation Act arising out of the accident and subsequent death of JUAN MENDEZ occurring on August 19, 2025. The Oklahoma Workers' Compensation Commission has scheduled a hearing to determine the legal beneficiaries of JUAN MENDEZ and to determine death benefits due the legal beneficiaries of JUAN MENDEZ. The hearing will be held on AUGUST 12, 2026 at 9:00 A.M. at the WORKERS' COMPENSATION COMMISSION, 1915 NORTH STILES AVENUE, OKLAHOMA CITY, OK 73105-4918 before JUDGE TARA INHOFE.
